24/01/2012: CONSULAR OUTREACH IN SEATTLE, WA (24-26 FEBRUARY)

PCG-SF "CONSULATE ON WHEELS" GOES TO SEATTLE, WA.

The Philippine Consulate General in San Francisco will conduct a consular outreach mission in Seattle, Washington on February 24-26, 2012.

When:       February 24, 25 & 26, 2012 (Friday, Saturday and Sunday)

Time:         beginning at 8:00 a.m.

Services to be rendered:

  • · Philippine passport issuance /renewal
  • · Dual citizenship
  • · Legal and notary services
  • · Overseas Absentee Voting Registration (new!)

Passport Services: Passport applicants are no longer required to bring photographs since they will have their pictures taken using the ePassport data capturing machines at the outreach site. However, they still need to bring other requirements, such as their current passport, the passport application form (which may be downloaded from the Consulate website - ) http://www.philippinessanfrancisco.org/uploads/pdfs/passport.pdf) , an application fee of $60.00 and a self-addressed stamped (Express Mail with tracking number $19.00) envelope or a prepaid courier envelope, with their application. Please submit photocopies of the supporting documents (e.g for passport applications submit a photocopy of the passport data/photopage).

Pre-submission of Applications: To facilitate the processing of passport applications, we urge applicants to submit their respective passport applications to the Consulate in advance via e-mail. Please see the instructions below:

  1. Get schedule/appointment slot from the Filipino Community of Seattle.
  2. After getting your appointment schedule/slot, download/fill-in the passport application form at http://www.philippinessanfrancisco.org/uploads/pdfs/passport.pdf (no need to attach photo).
  3. Send/e-mail the duly accomplished passport application form and a scanned copy of your old/existing passport to comms@philippinessanfrancisco.org (Please indicate that you are applying at the Seattle Outreach Mission.
  4. Please send your application not later than Tuesday, February 22, 2012.

Dual Citizenship: Please accomplish the application form (one set with photo) plus the supporting documents. Please submit a photocopy each of the document and the ID. Fee is US$50.   2. After getting your appointment schedule/slot, download the dual citizenship application form at http://www.philippinessanfrancisco.org/uploads/pdfs/Dual%20Citizenship.pdf (please attach the required photographs).
  3. Send/e-mail the duly accomplished application form and a scanned copy of the following documentary requirements:
    1. Copy of Philippine Birth Certificate and/or. Copy of Philippine Passport
    2. Copy of US Passport
    3. Copy of Marriage Certificate/Contract (for married women)
    4. Copy of US Naturalization Certificate

Notarial/Legalization of Documents: Bring Please bring the document and an original valid photo ID w/ signature (e.g. State ID), and a photocopy each of the document and the ID. Fee is US$25 per document.

Report of Birth: The birth of a child in the US should be reported/registered with the Philippine National Statistics Office through the Consulate General. See http://www.philippinessanfrancisco.org/uploads/pdfs/birth.pdf. This is needed if you wish to apply for a Philippine passport to a child born in the US of Filipino parent/s.  Please prepare 5 sets of the completed Report of Birth form and required documents.

Report of Marriage: Marriages between a Filipino and a foreigner must be reported/registered with the Philippine National Statistics Office through the Consulate General. See http://www.philippinessanfrancisco.org/uploads/pdfs/Report%20of%20Marriage.pdf. This is needed if you wish to apply for a passport using your husband’s surname for the first time (female applicants married to foreigners).  Please prepare 5 sets of the completed Report of Marriage form and the required documents.

The Philippine Consulate will only accept CASH, MONEY ORDER, BANK DRAFT or CASHIER'S CHECK (made payable to the Philippine Consulate General).
